Are you in network with my insurance?

We are in-network with many insurance companies (see below) but are also happy to see you out of network. The Physio Shop will bill any insurance company on your behalf, and our services are often 100% covered even for those who are out-of-network. The best way to check if we are in-network with your insurance company is to call the 800-number on the back of your insurance card. Your insurance company’s customer service department should be able to answer any questions you might have.

Do I need a referral?

Arizona is a “direct access” state for PT, which means you can always make an appointment for physical therapy without a doctor’s referral. Some insurance companies do require a referral before they will cover PT services, such as Medicare and Medicaid. When in doubt about your benefits, it’s best to ask your insurance company by calling the 800-number on the back of your insurance card.

What are deductibles and co-insurance?

A deductible is a amount set by your insurance that you must pay for your health care before the insurance company will start to pay on your behalf. Here’s a common example of how insurance deductibles are set up:

  •  Individual deductible (per calendar year): You pay up to $500
  • Family deductible (per calendar year): You pay up to $1000

Insurance payments kick in once either of these numbers is met, but at that point, your insurance may or may not cover all expenses–Some insurance plans also include co-insurance, which means that the insurance company pays a portion of your health care, and you also pay a portion, even once your deductible is met. For an 80/20 co-insurance rate, your insurance covers 80% of your health care costs, and you are responsible for 20%.

What is a co-payment?

A co-payment (“co-pay”) is a sum of money that you must pay to your health care provider at the time services are rendered, which is required by your insurance company. If you have a $20.00 co-pay, you will be responsible for paying that amount for each PT visit. At The Physio Shop, this amount can be paid at the time of service or we are happy to bill you for the amount at a later date if that makes life easier.
